Sports Physicals

Schedule a sports physical with a doctor to get cleared medically after June 1 of each school year.

Santa Rosa Community Health (SRCH) has a long history of caring for our youth.  SRCH has a clinic specifically designed to meet the needs of our teens. The teen clinic is the Elise Allen Clinic, located behind EA High School. We provide care at low or no cost for all Sonoma County teens. Even though we have just begun summer, this is a great time for youth to make an appointment to get their sports physical well in advance of when they will need it for fall. The clinic is open M-Th 8 am to 5 pm / 8-3:30 on Fridays please feel free to have your students call to make an appointment. Appointments: 707-583-8777

Go through your family doctor or use:

Before You Begin:

Make sure you have the following prior to attempting to register your child:

• Your child’s student identification number
• The exact spelling of your child’s first and last name as it appears in official school records
• A scanned copy of your child’s completed Pre-Participation Physical Exam on your computer (to be uploaded during registration)
• Your child’s active health insurance coverage information, including the name of the provider and the policy ID.

Creating an Account:

1. Go to https://sportsnethost.com/slater-parent
2. Click the Create an Account link near the bottom of the screen
3. Enter the required information and submit the form
4. Log in to the system with your email address and password
5. Follow the instructions inside the system

Registration Process

Be sure to carefully read the instructions on each screen and provide all required information. If you are unable to finish in a single session, the information you have already submitted will be saved and you can log back in at a later time to finish.

Important: The school will not receive your submission until you complete the entire process, ending with your final electronic signature on the last available screen. Be sure to check your email for a confirmation message after you have finished.

1. Find the coach for the sport you are interested in playing and email them about your interest. You can find the list of coaches below.

2. Ask the coach if Concussion Screening is required to play your chosen sport.

1. Listen for announcements during school for information on attending a meeting for the sport. 

2. Ensure you have all of the required documentation.

3. Attend the meeting.

Athletics Chain of Command

Occasionally students or parents have an issue, comment, or complaint they would like to see dealt with under the MHS Athletics Department. To make this situation fair and effective, there is an order of steps we request students and parents follow:

1. Contact Coach of specific team
2. Contact Head Coach of program
3. Contact Athletic Director
4. Contact Vice Principal in charge of Athletics
5. Contact Principal
6. Contact School District

We take issues, comments, and complaints seriously and will work to make sure all parties needs are met and satisfied to the best of our abilities.
